Objectives This study reports the effectiveness of combined Korean Medicine treatment, including Chuna manual therapy, for patients with chronic ruptured lumbar disc herniation (LDH) who were recommended for surgery. Methods Two patients with chronic extruded LDH received acupuncture, pharmacopuncture, herbal medicine, and Chuna manual therapy (flexion-distraction, myofascial release, and muscle energy technique) for six to seven weeks. Clinical outcomes were assessed using the Numeric Rating Scale (NRS), Oswestry Disability Index (ODI), and Pain-Free Walking Distance (PFWD). Results Both patients showed significant recovery. NRS scores decreased from 8 to 2, and ODI improved from 75.5% to 35.5% (Case 1) and 71.1% to 20.0% (Case 2). PFWD increased from 10m to over 700m. Analgesic injections were discontinued, and improvements were maintained during the follow-up period. Conclusions Combined Korean Medicine treatment incorporating Chuna manual therapy provides an effective non-surgical option for chronic ruptured LDH, leading to substantial pain relief and functional restoration even when surgical intervention was previously indicated.
