Objectives This study aimed to evaluate whether Facial Chuna Manual Therapy (FCMT) was associated with final remission and functional recovery in hospitalized patients with severe Bell's palsy. Methods This retrospective cohort study included 35 inpatients with severe Bell's palsy (House-Brackmann grading system (HBGS) ≥5) admitted between 2018 and 2021. Patients were classified into the chuna group and the no-chuna group according to whether FCMT was provided. The primary outcome was final remission, defined as a final HBGS ≤2. Secondary outcomes were assessed at three time points: final outcomes (final HBGS), discharge outcomes (inpatient length of stay and inpatient changes in HBGS and Sunnybrook voluntary movement), and outpatient follow-up outcomes (post-discharge changes in HBGS). Results Sixteen patients received FCMT and 19 did not. Baseline characteristics were generally comparable, although symmetry of voluntary movement was lower in the chuna group. Final remission occurred in 75.0% (12/16) of the chuna group and 42.1% (8/19) of the no-chuna group (risk ratio 1.78, 95% confidence interval 1.01-3.56; p=0.047). Discharge outcomes were generally similar between groups. Among patients with outpatient follow-up, post-discharge improvement in HBGS was greater in the chuna group. Conclusions In this small retrospective cohort, FCMT was associated with a higher final remission proportion and greater post-discharge improvement in HBGS in patients with severe Bell's palsy.
