Objectives This study aimed to evaluate the effect of chuna manual therapy for the management of shoulder-hand syndrome (SHS). Methods This study was conducted across 10 electronic databases, including PubMed, The Cochrane Library, Embase, China National Knowledge Infrastructure (CNKI), Research Information Sharing Service, Korean studies Information Service System, DBpia, Oriental Medicine Advanced Searching Integrated System, National Digital Science Library, KMbase up to October 2024. Randomized controlled trials (RCTs) investigating the effect of chuna manual therapy for SHS were included. The risk of bias was assessed using the Cochrane risk of bias tool. Results After screening 8 RCTs included and meta analysis was conducted with 4 RCTs that reported visual analogue scale (VAS), Fugl-Meyer assessment (FMA), modified Bathel index (MBI). As results, chuna manual therapy with modern rehabilitation showed significant improvement in VAS (standardized mean difference, SMD; -1.35 [95% confidence interval, CI; -1.94 to -0.76], p<0.00001, I2=69%), FMA (SMD 1.93 [95% CI, 0.95 to 2.91], p=0.0001, I2=90%), MBI (SMD 1.09 [95% CI, 0.43 to 1.75], p=0.001, I2=69%). Conclusions Chuna manual therapy was effective for SHS. Despite favorable outcomes in the meta-analysis, the evidence supporting the effect of chuna manual therapy for SHS remains limited due to reliance on a single Chinese database (CNKI) and the methodological limitations of the included studies. Well-designed, high-quality RCTs are necessary to confirm these findings and address the identified limitations.
